As mentioned on our appointment request form, we ask for your first name and phone number (both are required for us to contact you), and although you are able to include your email address we have left that as an optional form field. The reason the email address form field is optional is because some people use their work email address, and some employers monitor usage of employee email usage.

Comments and user registration are disabled throughout our website. Thus, we do not set cookies in these capacities.

What Rights You Have Over Your Data

Comments and user accounts are not active on our website. Thus, we do not maintain any data on you in those capacities. When submitting an appointment request, we ask for your first name, email address and phone number. That information is not retained online. In addition, we do not ask for any personal medical information.

When submitting an appointment request, the limited data we ask for is transmitted securely from our website to our internal staff, and utilized to respond to your request and schedule an appointment.
